Hybrid substitutes
Hybrid substitutes are removable substitutes anchored to the remaining teeth with slide-in joints, possibly with the system of double-crowns (so called telescopes).
They represent the most suitable and, from the viewpoint of aesthetics and use, the most comfortable option of a removable substitute. The anchoring of the substitute is possible with a slide-in link of variously modified shapes; the principle of so called matrix and patrix, for instance, it is the principle of bachelor’s button. The crossover of the substitute onto the remaining teeth is almost invisible. Also, the stability of the substitute is very good.
A condition for this treatment is the necessity to set the teeth neighboring the gap with an artificial crown; it bears one part of the slide-in link. At the same time, activation is necessary, the bone and mucosa, as under each removable substitute, change in the time before pressure decreases. Therefore it is necessary to regularly check the stability and balancing of the denture and run a so-called Rebase (refill of resin into the denture) because of the above mentioned decrease in the bone and the worsening of the stability of the denture.
Disadvantages:
- necessity of activation – a part of the slide-in link in the denture gets worn out after some time and it needs to be replaced
- necessity of a so-called rebase (refill of resin into the denture at the point of the seat of the denture)
= permanent investment, financially more demanding choice of treatment.
Advantages:
- aesthetically the most comfortable option for a removable substitute (crossover of the substitute onto one’s own teeth is almost invisible)
- very good stability and retention (anchoring of the substitute) of the denture in comparison to a complex removable substitute
- under certain circumstances they represent a continual crossover between the treatment of a toothless jaw with implants (fixed type of treatment) and with complex removable substitutes. The denture itself is applied onto the remaining specially adjusted teeth (very frequently two cynodonts, the teeth have various attachments or links in the shape of a ball, strap, etc.)
